Proposals must include:

  • Title
  • Type of presentation format (breakout, roundtable, interactive workshop, poster)
  • List of presenters and affiliation for each with lead author and non-presenting authors identified
  • Contact information (address, phone, email and/or fax) for all presenters
  • A bio-sketch of 2000 characters or less for each presenter to be included in the conference abstract and contact book
  • An abstract of 500 characters or less
  • At least one learning objective (what knowledge or skills will be gained from the session?)
  • A signed Presenter Disclosure form (downloadable from our website)
  • A curriculum vita or resume for each presenter
